This is a unique opportunity to shape the future of community health by building robust chronic disease management programs, strengthening care coordination, advancing clinical quality initiatives, and empowering a diverse team of healthcare professionals to deliver exceptional, patient-centered care to our underserved community.Key Responsibilities:Clinical Leadership Team DevelopmentProvide leadership through the Nursing Supervisor to staff RNs, including performance management, scheduling and staffing optimization, professional development planning, and departmental budgetingSupport and develop the Medical Assistant team through the MA Supervisor to improve clinical workflowsSupervise Community Health Workers to expand outreach initiatives and strengthen SDOH (Social Determinants of Health) screening and supportLead the Clinical Quality Coordinator in addressing chronic disease gaps and preventive care initiativesProgram Development Care CoordinationDesign and oversee comprehensive chronic disease management programs that improve patient engagement, satisfaction, and health outcomesLead patient education initiatives and integrate remote monitoring technologiesManage transitional care programs, including: post-hospital discharge follow-up, coordination with home health services and reduction of avoidable hospitalizations and ER visitsQuality Improvement Population Health ManagementProvide leadership for clinical quality improvement initiatives aligned with HEDIS and UDS performance metrics and value-based care requirementsDrive population health strategies across patient panelsCollaborate with clinical teams to analyze quality metrics, implement evidence-based interventions, and monitor progress toward quality benchmarksOperational ExcellenceOptimize supply chain and inventory management across sitesImplement and sustain Lean 5S systems for efficient clinic operationsCollaborate with interdisciplinary teams to advance quality metrics and care coordination strategiesQualifications:RequiredActive RN license in South Carolina (or willingness to obtain SC RN License)5–7+ years of registered nursing experience3+ years of proven clinical leadership/management experienceStrong knowledge of:Quality metricsCare coordinationInterdisciplinary team leadershipPreferredExperience in case management and/or chronic disease programsBackground in supply chain or inventory managementPediatric nursing experience, including familiarity with the Vaccines for Children programExperience with clinical quality improvement and population health initiativesCompensation & Benefits:Affinity Health Center offers a comprehensive and competitive benefits package, including:Fully funded health insuranceGenerous paid time offGroup health, life, dental, vision, and disability coverageHealth Reimbursement Account (HRA)403(b) retirement plan with 3% employer match12 paid holidaysThe opportunity to make a meaningful impact in community health and serve populations who need it mostWhy This Role Stands Out:High-impact leadership role in a mission-driven FQHC environmentOpportunity to build programs and shape strategyDirect influence on community health outcomesJoin Our Mission:If you are a passionate nursing leader ready to transform care delivery, build high-performing teams, advance clinical quality, and promote health equity in our community, we want to hear from you!
