Nurse Practitioner - Post Acute Care - North Region

Position Highlights:• Position: Nurse Practitioner (NP) - Post Acute Care• Location: Skilled Nursing Facilities in Endeavors north region (Evanston, Swedish Hospital).• Full Time/Part Time: Full-Time, 40 hours per week• Hours: Monday - Friday between 8:00 a.m. and 5:00 p.m. • Potential for weekend hours in the future• No evenings or holidaysWhat you will need:• License: Nurse Practitioner (APN) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required• Education: Completion of Master's level program in specialty area• Certification: BLS/CPR certification required• Experience: Minimum of three years Nurse Practitioner experience. Experience with advanced directive and goals of care.
