Local Contract Nurse Practitioner - Primary Care - $80 per hour

CTC Medisource is seeking a Nurse Practitioner Primary Care for a local contract job in Chagrin Falls, Ohio. Job Description & Requirements Specialty: Primary Care Discipline: Nurse Practitioner Duration: 26 weeks 40 hours per week Shift: 10 hours Employment Type: Local Contract Location: 16801 Chillicothe Rd., Chagrin Falls, OH, USA, 44023 Schedule: Monday-Friday 8:30am-7:30pm, Sat 9am-5:30p, Sun 9a-4:30p. This is a FLOAT position which will float to: Chardon, Painesville, Willoughby, Lyndhurst, UH, Mayfield, Woodmere, Bainbridge, Twinsburg, Aurora, Hudson, Westlake, Strongsville, Hinckley, Parma, Lakewood, North Olmsted, Northfield. Skills & Experience A MINIMUM of 1 year of recent Primary Care experience as a Nurse Practitioner in either a family medicine, primary care practice, or urgent care is required. Working knowledge and understanding of quality measurement in the management of chronic disease conditions Duties Provide Primary and Family evidenced-based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counselling, and disposition planning for our patients. Provide administrative duties to include but not limited to opening/closing the clinic, inventory, patient follow up, verifying insurance and collecting payment, maintaining a clean environment. Be responsible for handling patient appointments and performing routine examinations to ensure positive patient health outcomes Perform duties including speaking with patients about healthy lifestyle choices, completing diagnostic tests to help diagnose and treat patients for injuries or illnesses and coordinating with other healthcare professionals to maintain a comprehensive plan of care Encourage patients to become active participants in their own healthcare and well-being through services such as health screenings, behavioral health and wellness, chronic condition management Education Master's Degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role CTC Medisource Job ID JPC74765.
