Recognized as one of New York’s fastest‑growing IPAs, NSIPA supports improved clinical outcomes, enhanced provider performance, and cost savings for payers through its robust infrastructure and operational expertise. Position Overview: We are seeking a highly organized and detail‑oriented Chronic Care Coordinator (Medical Assistant) to support our care coordination team. This non‑clinical, remote position focuses on patient outreach, documentation management, and chart review. The ideal candidate brings experience in chronic care documentation, strong communication skills, and proficiency with electronic health record (EHR) systems. This is a fully remote role.Roles & Responsibilities:To perform this job successfully, an individual must be able to perform each essential function satisfactorily. Reasonable accommodation may be made to enable qualified individuals with disabilities to perform the essential functions.·        Accurately upload and manage patient documentation within EHR systems·        Conduct thorough chart reviews to ensure accuracy, completeness, and compliance.·        Identify and report missing or outdated documentation to the appropriate care team members.·        Communicate professionally with patients to support continuity of care, follow-ups, and coordination needs.·        Collaborate with clinical staff to maintain current and accurate patient records.·        Ensure full compliance with HIPAA and internal confidentiality standards.·        Bilingual in English and Spanish is a bonus. Minimum Requirement and Qualifications:·        Minimum of 1 years of experience as a medical assistant.·        Prior experience with chart review, documentation, and chronic care management is mandatory.·        Proficiency with EHR/EMR systems.·        Strong attention to detail, organization, and time management skills.·        Ability to work independently and manage remote work flows effectively.·        Knowledge of CMS chronic care management billing codes (preferred)·        Personal computer and reliable high speed internet connection is required, and equipment is not provided for this role.Physical Demands:The physical demands described here are representative of those an employee encounters while performing the essential functions of this job.
