Job Overview We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care in various settings, including hospitals, nursing homes, and outpatient facilities. This role requires excellent clinical skills, the ability to work collaboratively with other healthcare professionals, and a passion for supporting patients and their families. Duties As a Licensed Practical Nurse or Registered Nurse, your responsibilities will include: Providing direct patient care under the supervision of registered nurses and physicians. Administering medications and treatments as prescribed, including IV infusion and injections. Conducting patient assessments, monitoring vital signs, and documenting medical records accurately. Assisting with daily living activities for patients in long-term care, assisted living, or home care settings. Collaborating with interdisciplinary teams to develop and implement individualized care plans. Supporting patients with developmental disabilities, Alzheimer’s care, dementia care, and memory care. Performing procedures such as catheterization, tube feeding, phlebotomy, and specimen collection. Ensuring compliance with HIPAA regulations and maintaining patient confidentiality. Participating in discharge planning and providing health coaching to patients and families. Requirements To be considered for this position, candidates must meet the following qualifications: Valid LPN license in the state of practice. Experience in critical care, acute care, or long-term care settings is highly desirable. Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks. Knowledge of medical terminology and ICD coding (ICD-9/ICD-10). Strong skills in patient assessment and basic life support (BLS). Experience working in various specialties including pediatrics, geriatrics, emergency medicine, or hospice care is a plus. Excellent communication skills and the ability to work effectively within a team environment. A commitment to ongoing professional development and adherence to best practices in nursing.
