Recovery Program Coordinator

PCHC is looking for a Recovery Program Coordinator to support 2 clinics in Bangor - Penobscot Community Health Center and Hope House Health & Living Center! Schedule: Full-Time, Monday-Friday, 8am-5pm (NOTE: Schedule may be negotiable depending on final candidate's preferences.)
Highlights of the position:
Embodies PCHC’s mission, vision, and values by treating patients with kindness, respect, and dignity.
Serves as a primary point of contact for matters related to the Recovery Program at the clinics. Supports patients throughout Recovery Program process and identifies and provides resources as needed.
Answers phone calls and requests related to the program.
Reviews program requirements with new patients in recovery and provides education on what to expect.
Assists with intake paperwork and additional resources such as goal setting, recovery skills plans, and more.
Works with patients to identify and eliminate barriers to maintaining attendance.
Meets with patients who are out of compliance with program requirements to discuss plan and determine next steps.
Reviews, maintains, and distributes reports to identify screens that are due, compliance issues, and/or areas of needed support.
Contacts patients for urine drug screens and film counts as per protocol and as requested by patient’s care team.
Orders patient labs and obtains initial urine drug screens as necessary.
Tracks and records patient attendance for group therapy appointments.
Performs patient conformation and no-show calls for counseling and group therapy appointments.
Collaborates across teams to share best practices and improve patient experience as it relates to recovery.

Education and Experience:
Required:
High School Diploma or equivalent required.
2 years’ experience working in an outpatient healthcare setting required.
Will have up-to-date training and certification in Healthcare Provider Basic Life Support issued by American Heart Association, American Red Cross or American Health & Safety Institute. Must obtain within 2 weeks if not current at hire.
Preferred:
BS in Social Work, Human Services, Drug and Alcohol Counseling OR graduate of an accredited program for Medical Assisting preferred.
Graduate of an accredited program for Medical Assistants, or CMA- Certification by the AAMA, or RMA- Certification by the AMT, or CCMA- Certification by the NHA. Or prepared to certify within 60 days of hire
