The role requires clinical interviews, diagnostic formulation, and treatment planning while coordinating care and managing referrals according to program standards.Key ResponsibilitiesComplete initial comprehensive assessment of clients who have identified a need for behavioral health services. Assessment requires clinical interview and contact with collateral providers/natural supports/family members when possible and appropriate and consent is provided.Complete diagnostic formulation and treatment recommendations.Engage client in safety/crisis planning.Provide crisis intervention and de-escalation when indicated.Assess eligibility for services, medical necessity, and treatment preferences.Complete treatment plans including client's strengths, preferences, and behaviorally based goals and objectives as required by program standards.Complete referrals to indicated services and coordinate care on behalf of client, as required by program standards.Document service delivery activities according to program standards.Education RequirementsMust have permanent MA license or have a Master's degree and working towards independent MA license (LMFT, LCSW/LICSW, LMHC, PsyD)Required Experience1 year of recent experienceWork Environment / Physical RequirementsMay be required to travel and transport patients in your personal vehicle depending on position.Must be willing to float to all CHL locations. There is a 35 mile radius between them.Must be able to travel to all locations within 35 mile radius; advanced notice will be given for assignment location each day.Flexibility is key with this position, as the hours could change.Must be willing to work as needed; 40 hours per week is guaranteed.Flexibility is key regarding weekend requirements; willing to work as needed.Important NotesPreference will be given to those with no Return to Office (RTO), but applicants can submit with up to 5 days of RTO.Can submit with 5/11 start dates, but preference will be given to those with 5/4 start dates.
