Position Summary
The Registered Nurse (RN) delivers skilled nursing services to individuals of all ages (pediatric, adult, and geriatric) receiving home and community-based services throughout South Carolina. Working within the scope of professional nursing practice and in accordance with physician orders, the RN provides comprehensive clinical assessments, develops and oversees individualized care plans, administers medications and treatments, monitors health conditions, and responds to changes in status.
The RN provides clinical oversight and guidance to Licensed Practical Nurses (LPNs), Direct Support Professionals (DSPs), and other care team members as applicable. The RN supports individuals with Intellectual and Developmental Disabilities (IDD) and other complex medical needs, promoting safety, dignity, independence, regulatory compliance, and an optimal quality of life.
Position Scope
This position operates within the Nursing Department and provides direct and indirect clinical services in home and community-based settings. The RN performs comprehensive assessments, develops and updates individualized care plans, and ensures services are delivered in accordance with physician orders, Medicaid waiver requirements, and South Carolina nursing regulations.
The RN practices independently within the scope of South Carolina licensure and provides clinical guidance to LPNs, DSPs, and caregivers as appropriate.
Key Responsibilities
The Registered Nurse (RN) conducts nursing assessments, develops and monitors individualized care plans, and provides direct skilled nursing services in accordance with physician orders and regulatory requirements. Responsibilities include administering medications and treatments, monitoring vital signs and health status, providing wound care, and supporting specialized medical needs such as PEG/G-Tube or tracheostomy care as applicable. The RN collaborates with physicians, families, and interdisciplinary teams to ensure coordinated, high-quality care, provides clinical guidance and caregiver education, maintains accurate and timely documentation, and ensures compliance with all state, federal, and agency standards. The RN also participates in care coordination, quality improvement initiatives, and additional duties consistent with licensure and organizational needs.

The RN must maintain strict confidentiality of all participant information in compliance with HIPAA and OneWell policies. Protected Health Information (PHI) may only be accessed, used, or disclosed as required for treatment, payment, or healthcare operations.
Documentation Requirements
Completion of nursing assessments and reassessments
Care plan development and updates
Medication Administration Records (MAR) oversight
Incident documentation and follow-up
Clinical notes in electronic health record (EHR) systems
Compliance with Medicaid and regulatory documentation standards
Certifications
Active South Carolina RN License (required)
Multi-State Compact License (preferred, if applicable)
CPR/BLS Certification (required)
Current TB test or screening (required)
Additional certifications based on case assignment
Education
Bachelor of Science in Nursing (BSN) preferred
Associate Degree in Nursing (ADN) accepted if licensed and experienced
Experience
Minimum one (1) year of nursing experience required
Experience in home health, IDD services, or long-term care preferred
Experience with G-Tube, tracheostomy, ventilator care preferred (case dependent)
Technical Skills
Strong clinical assessment skills
Medication administration proficiency
Wound care and infection control knowledge
Ability to interpret physician orders
Proficiency with electronic documentation systems
Basic computer and mobile device proficiency
Languages
English required. Bilingual abilities preferred (not required).
Driver License
Valid driver’s license required. Reliable transportation required. Ability to travel to participant homes within assigned region.
Travel Requirement
This position requires local travel within South Carolina to participant homes and community settings. Travel percentage varies depending on caseload and geographic assignment.
