Job Description
The Heritage Center of Holiday Health Care is seeking a dedicated and reliable Full-Time MDS Nurse to join our team! This important role will focus on completing MDS assessments and participating in resident care conferences to ensure high-quality care for our residents.

Key Responsibilities:
Complete and submit MDS assessments for all nursing unit residents
Participate in and coordinate resident care conferences to ensure optimal care plans
Conduct unit rounds to assess resident function and team interaction
Ensure MDS is submitted on time in compliance with State and Federal guidelines
Collaborate with the health care team, including physicians, to create an interdisciplinary care plan for each resident
Provide education to residents, families, and staff regarding the care provided
Monitor the completion of ADL (Activities of Daily Living) assessments
Ensure all care plans are aligned with the MDS assessments and RAP guidelines

Qualifications
Graduate of an accredited school of nursing
Active nursing license in the State of Indiana
Experience in long-term care nursing (preferred)
Working knowledge of the RAI manual, ICD-10 codes, case mix index, and strong assessment skills
Proficient with computers and MDS submission processes
Strong interpersonal and communication skills
