Join our Social Service team and become a compassionate Licensed Social Worker (LSW) making a lasting impact on patients and your career.Qualifications:Master’s degree in Social Work preferred, Bachelor's degree in Social Work with a minimum of 1 year of healthcare experienceMinimum of two years social work experience in a healthcare setting or social services agencyCurrent Social Worker license (required)Hospice experience a plusKnowledge of and ability to obtain community resources (information and referral, state/federal financial programs and eligibility requirements, etc.)Understand Hospice philosophy, principles of death/dyingStrong organizational and communication skillsValid driver's license (required)Valid Auto Insurance (required)Responsibilities: Provide wide range of psychosocial services to terminally ill patients and their familiesResponsible for making contact with patient, family, and caregiver within 48 hours of admissionCase Manager dutiesPsychosocial assessment must become part of patient's chart within 24 hours of completionRegularly assess the patient and family psychosocial needsRegularly assess the pre-bereavement/anticipatory grief needsImplement the Plan of Care provisions to meet their needs, which include: social service goals for alleviating problems, supportive counseling, problem solving, community referrals pre-bereavement and bereavement careRegularly re-evaluate the patient's and family's psychosocial needsParticipate in weekly interdisciplinary team meetingsAssist with making funeral arrangements if requestedProvide counseling during the process of the terminal illness and at the time of death if requestedBenefits:Competitive salaryCompany car with fuel and insurance coveredCompany cell phoneComprehensive health, dental, and vision insuranceGenerous PTO accruing immediately401(k) with company matchingApply Now: We make it easy for you to apply and join our team.
