Home Health Coordinator - New Milford, CT
The Home Health Coordinator is responsible for scheduling and coordinating patient visits across disciplines to ensure timely, efficient, and compliant home health service delivery. This role supports clinician productivity, episode management, and patient satisfaction while working closely with clinical teams, patients, and families.Location: On-site / New Milford, CT OfficeSchedule: Full-Time / Monday - Friday / 8:30am -4:30pmKey Responsibilities:Scheduling & Care CoordinationMaintain accurate patient schedules, ensuring all ordered visits are appropriately assigned across disciplinesSchedule all Start of Care (SOC) and Resumption of Care (ROC) visits in the scheduling systemAssign and maintain a primary case manager for each patientCoordinate clinician coverage for absences, including vacations and sick timeCommunicate with patients and families regarding visit schedules and changesEpisode Management & ComplianceEnsure visits are completed timely to support billing, compliance, and quality outcomesReconcile scheduled visits against physician orders to ensure regulatory adherenceMonitor recertification timelines and provide weekly reminders to clinical staffCollaborate with clinical managers to support effective episode managementSupport scheduling strategies to minimize Low Utilization Payment Adjustments (LUPAs)Maintain working knowledge of Medicare guidelines and payment modelsProductivity & Operational OversightMonitor clinician schedules to ensure productivity expectations are metRun and analyze reports related to visit completion, timeliness, and scheduling accuracyIdentify scheduling gaps, inefficiencies, or risks and proactively resolve themPartner with leadership to ensure balanced coverage across service territoriesCommunication & CollaborationServe as a liaison between patients, families, clinicians, and leadershipCommunicate scheduling updates and important information to field staffCollaborate with interdisciplinary teams to ensure coordinated, patient-centered careQualifications2+ years of healthcare experience required, including medical terminology.Bachelor's degree or licensed or experienced high level functioning Medical Assistant, CNA, Coder, or other Health care personnel experience.Minimum one year of community health experience preferred, with a strong knowledge base of home health or hospice and associated regulations.Knowledge of Medicare guidelines, episode management, and LUPA implications preferredStrong organizational, multitasking, and problem-solving skillsProficiency with EHR and scheduling systemsExceptional customer service skills including phone manner and ability to build relationships in a fast-paced environment.Why RVNAhealth?RVNAhealth (formerly known as RVNA, the Ridgefield Visiting Nurse Association) is a twelve-time winner of the Hearst Media Top Workplaces Award. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
