MSW Social Worker
Job Summary: We seek a highly skilled and experienced Registered Nurse with specialties in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, and Operating Room. The Registered Nurse will provide skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Key responsibilities include patient assessments, medication administration, treatment planning, and collaboration with the interdisciplinary team. Strong communication and collaboration with patients, providers, and team members are critical. A minimum of 2 years of experience and exceptional skill is required. Responsibilities:Perform patient assessments and monitor patient conditions.Administer medications and manage treatment plans.Collaborate with interdisciplinary teams to coordinate patient care.Provide case management and hospice care services.Communicate effectively with patients, families, and healthcare providers.Document patient care accurately and timely.Required Skills & Certifications:Active Registered Nurse license in the state of practice.Bachelor's degree in Nursing or related field.Minimum of 2 years of experience in hospital, home health, or hospice care.Clinical assessment, medication administration, and treatment planning skills.Excellent communication and interpersonal skills.Driver's license, car insurance, and automobile for field work.Preferred Skills & Certifications:Experience in acute care or case management/discharge planning in hospital settings.Specialty certifications related to ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room.Special Considerations:Floating to different units as required.Weekend work requirements to be discussed during interview.Requested Time Off (RTO) limited to no more than 7 days.Facility type includes acute care and case management/discharge planning hospital environments.Scrub color requirement: Navy or Black.Scheduling:Start date: ASAP.Floating schedule with variable unit assignments.Weekend shifts subject to discussion during interview.
