As the Care Management Lead, you will serve as the point person for Case Management Services alongside the Clinical Site Director, managing referrals and overseeing team performance. You will provide administrative supervision that includes monitoring progress notes, timekeeping, productivity measures, and case assignments to ensure the team operates at its highest potential.Staff Development & Mentorship. You will actively support the Site Director in recruiting, coaching, mentoring, and developing team members, including onboarding new staff. Fostering a spirit of teamwork, unity, and psychological safety — one that embraces diversity, resolves conflict constructively, and helps every employee succeed — is a cornerstone of this role.Integrated Care Coordination. You will provide individualized care coordination across healthcare, behavioral healthcare, and non-healthcare services, ensuring that each client's biopsychosocial needs are met holistically. This includes therapeutic behavioral services, collateral contacts, and services delivered in all settings that meet the client's needs.Community Outreach & Navigation. You will build and maintain relationships with community partners who may refer clients to our services and provide direct support to clients in navigating The Centers' systems. You will actively link clients to health promotion, wellness activities, and community resources.Clinical Documentation & Compliance. You will perform essential administrative functions including the completion of clinical records, tracking of billable hours, and ensuring the team's progress notes are completed within 72 hours of service.What We Are Looking ForCore CompetenciesClinical Acumen.
