This role involves managing patient care plans, performing start of care and recertifications, and conducting regular patient visits. Candidates must have a current driver's license and auto insurance, and must follow a radius rule of 50 miles for employment eligibility.Responsibilities:Perform start of care and recertification visits.Conduct 5-6 daily visits without additional travel or admission assignments.Provide safety evaluations or teaching visits in hospice situations if needed.Work collaboratively with support from the Clinical Coordinator and Director.Requirements:Current BLS certification and ND Physical Therapy License.Preferred experience in Home Health and OASIS training.Experience managing a workload of 6.5 points per day.Black, navy blue, or grey scrubs are required.Client Details Address 986 2nd Ave W City Dickinson State ND Zip Code 58601
