MDS Coordinator for small LTC Facility with Sign on Bonus

Alliance Health at Baldwinville - Alliance Health at Baldwinville is hiring an experienced MDS Coordinator to manage resident assessments and care planning at our quaint, 95 bed, skilled nursing/long-term care facility. As the backbone to our multidisciplinary care approach, you have an opportunity to make a real difference in our Residents' lives. Due to our continued growth, we are looking for team-oriented individuals to join our clinical team.The MDS Coordinator Nurse will be responsible for, but not limited to:Completion of the OBRA, MDS and CAA's per scheduleParticipate in the development and ensured completion of the Care Plan, identifying the individual needs of the residents.Facilitate weekly managed care meetings, participate in weekly care plan meetings, and other meetings as assigned.Assist with monitoring of resident documentation.Assist in ensuring C.N.A documentation is reflective of resident care.Ensure accurate coding and supporting documentationMaintain CMS and MA complianceSupport audits and survey readinessQualifications:Must have active nursing license in MA and one to two years of MDS experience working in an LTC or Sub-acute setting preferred.MDS experience preferredLong-term care experienceStrong documentation and organizational skillsCompensation and Benefits$90,000 – $102,000 per year (based on experience)$5,000 Sign-On BonusHealth, dental, and vision insurancePaid time off & holidaysEmployer paid Life insurance401(k) with employer matchEmployee Assistance ProgramTuition ReimbursementSupportive team environment#AHMS HP - 3-6-2026
