Responsibilities include: Managing chronic and preventive care Developing personalized care plans Coordinating with specialists and care team Meeting quality metrics in value-based model Building strong patient relationships Using EMR (Epic) for documentation Qualifications MD or DO required Internal Medicine, Family Medicine, or Geriatrics Board Certified or Board Eligible (required) Florida license or eligible 2-6 years of experience preferred (new grads welcome) Interest in value-based or senior care preferred
