Integral Care Home Care - Home care Field RN Superviser - Per-DemHome care experiance a Must.Qualifications:• Valid & current New Jersey Registered Nurse License and Registration• Current Driver's License• Organized and Patient oriented• Able to work independently• Good oral and written communication skills• Minimum of 1-2 years clinical nursing experienceResponsibilities: Field Nurse Superviser• Assess patient at start of care to develop a plan of care• Initiate Plan-of-Care and make necessary changes as patient's needs evolve• Evaluate patients in their homes• Instruct and advise patients and/or families as well as the home health aides in patients' care• Reassess patients every two months to determine any condition change as well as supervise the home health aide to ensure proper careBenefits• Hourly rate of $50-$100• $100/hour for Initial assessment & reassessments• +$25 for after-hours• Office time $50/hour• Visits to HHA $75.00/hour• Weekend Visits - $75/hour• On Call - $50 a day - in case there are calls.• Mileage Reimbursed
