Organization: Rainbow Omega, Inc.Program: Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID)Employment Type: Full-Time /Non-ExemptReports To: ICF Enrichment Coordinator Position Summary The Life Skills Coach / Activities & Vocational Support staff member provides direct support to adults with intellectual and/or developmental disabilities by assisting with daily living skills, implementing active treatment plans, facilitating meaningful recreational activities, and supporting pre-vocational and vocational skill development. This position promotes independence, engagement, dignity, and quality of life in accordance with each resident's Individual Program Plan (IPP), behavior support plans, and applicable ICF/IID regulations. Direct Support & Life Skills Development Assist residents with activities of daily living (ADLs) as neededSupport independent living, social, communication, and self-advocacy skillsKnow each resident personally to support individual needs and preferencesFill in as a resident aide as needed to ensure continuity of care Activities & Active Treatment Implement activity portions of active treatment plans for ICF/IID residentsPlan and deliver fun, safe, and engaging daily activities, including weekendsAssist residents with activities outlined in the IPP and Activities BinderDevelop and display thematic and holiday decorations throughout the facility Pre-Vocational & Vocational Skills Development Plan and implement pre-vocational and vocational training activitiesTeach job-related skills including task completion, time management, communication, and teamworkAssess resident interests, strengths, and support needs related to vocational participationIncorporate vocational goals into residents' IPPs in collaboration with the interdisciplinary teamCoordinate on-site vocational activities and community-based vocational or volunteer opportunitiesDocument participation, skill acquisition, and progress toward vocational goals Community Integration & Transportation Transport residents to on-campus and off-campus activities using facility vehiclesTransport residents to medical, vocational, and personal appointmentsShop with and for residents in the local community to promote independence Documentation & Compliance Prepare documentation related to IPPs, behavior support plans, and vocational goalsComplete reports required by licensing and regulatory entitiesAccurately document daily activities and resident progress Communication & Team Collaboration Report complaints, incidents, or concerns to the Program Director or Program CoordinatorParticipate in ICF/IID and Rainbow Omega meetings or committees as requestedCollaborate with interdisciplinary team members and external partners Administrative & Other Duties Operate office equipment including computer, printer, and copierSubmit supply requests to the Program CoordinatorPerform other duties as assigned Qualifications Required:High school diploma or GEDAbility to work effectively with adults with intellectual and/or developmental disabilitiesValid driver's license with acceptable driving recordPreferred:Experience working in an ICF/IID or vocational settingCoursework in human services, vocational rehabilitation, or related fieldCPR, First Aid, CPI certification or willingness to obtain Physical & Work Requirements Ability to assist with lifting, transferring, and mobility supportAbility to stand, walk, bend, and engage in physical activities throughout the shiftWillingness to work evenings, weekends, holidays, and flexible schedulesPowered by JazzHR
