This role is designed for nurses who excel in patient engagement, education, and coordinated virtual care delivery.ResponsibilitiesDeliver scheduled chronic care outreach and patient check-insReinforce care plans, treatment goals, and self-management educationMonitor RPM data and identify trends requiring clinical interventionCoordinate follow-up care after hospitalizations or specialty visitsCommunicate effectively with interdisciplinary care teamsDocument all patient interactions accurately and compliantlySupport adherence by identifying and addressing patient barriersRequired Qualifications Active Virginia LPN/LVN license in good standingAt least 2 years of nursing experience (chronic care or telehealth preferred)Experience with CCM, RPM, or value-based care models is a plusStrong clinical assessment and patient communication skillsComfortable working independently in a remote clinical role
