JOB PURPOSEProvides each of the assigned patients with routine daily nursing care and services in accordance with the patient's assessment and care plan, and as directed by the nurse supervisor.KEY RESPONSIBILITIESProvides care ensuring patient/resident safetyApplies and releases restraints, and provides exerciseAssists residents in and out of bed and into wheelchair with the correct use of the transfer belt and practices proper body mechanics while moving or transferring patientsAssists residents in bathing: to include bed baths, tub baths, and showersAssists residents in dressing or undressing, and personal grooming e.g., oral/denture care, brushing hair, trimming fingernails and toenails, skin care and shavingAssists residents to and from activities as requestedAssists residents to dining area for meals; passes food trays to individual residents in accordance with diet cards on tray and needs; documents intake of nourishment, and can identify the four basic food groups on a sample breakfast, lunch, and dinner trayAssists with admissions, discharges, and transfers as requested; understands and utilizes care plansCleans bedpans and empty catheter bags, provides external/indwelling catheter care, provides incontinent care as needed for residentsOffers fluids at appropriate times (including routine ice and water rounds) and distributes fresh water to patients and picks up used water pitchersProperly documents in accordance with established guidelines; receives and gives reports on patient's status or condition and notifies nurse of any changes in resident conditionReports skin irritation around ostomy bag to the Charge Nurse, reports problems with ostomy bag to the Charge NurseProvides range of motion exercises for residentsProvides routine care for the bedfast patient; turns residents intermittently if patient is bedridden due to illnessRecognizes and reports signs and symptoms of abuse to Charge Nurse/Administrator, and identifies and reports changes in condition to Charge NurseRemoves bedding and replaces with clean linens per schedule or condition, and as neededTakes vital signs of patient, e.g., temperature, blood pressure, and respiration and records information and documents or reports information regarding the patient's health, I/O, any change of medical condition, and care givenKNOWLEDGE, SKILLS, ABILITIESCommunicates well with patients/residents/clients and family members providing warm and friendly greeting and an approachable demeanor to families, visitors, patients/residents/clientsResponds to expressed concerns displaying a helpful, caring demeanorAnswers questions when appropriate in a professional mannerParticipates in center/agency surveys (Licensure / JCAHO) and any subsequently required reportsAttends and participates in continuing educational programs to keep abreast of changes in your fieldAttends and participates in mandatory in-servicesHonors patients/residents' rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaintsComplies with corporate compliance programReports job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessaryFollows established safety regulations, to include fire protection & prevention, smoking regulations, infection control, etc.Follows established safety procedures when performing tasks and/or working with equipmentPerforms other related duties as necessary and as directed by nurse supervisorMINIMUM EDUCATION REQUIREDSome high school or high school diploma or GED requiredMINIMUM EXPERIENCE REQUIREDOn-the-job training providedMINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAWTemporary Nurse Aide must not have adverse findings on criminal background checks. Temporary Nurse Aide must be able to demonstrate competency to provide nursing services consistent with CMS waiver requirements.ADDITIONAL QUALIFICATIONSHigh school graduate preferred. Previous experience preferred.
