LPN PositionS:US is seeking an LPN for our men's shelter in Brooklyn.This position is located at 599 Ralph Avenue Brooklyn, NY 11233.LPN meets with the Program Director, Director of Social Services, Registered Nurse, and Social Service staff to coordinate shelter residents to receive ongoing treatment in the community.In this position you will:Take vital signs, such as blood pressure, temperature and weightAssist with diabetes medication and treatmentTake medical histories and enter information in computer systemsCoordinate with Case Managers, Senior Case Managers and Director of Social Services to ensure proper care, nutrition and treatment plans are followedFollow up with community health care providers to ensure medical treatment plans are documented, monitored and followedHave flexibility to work a schedule that includes evenings and weekendsQualifications:New York State Licensed Practical Nurse Experience working on a multi-disciplinary team and/or previous experience with homeless population Bilingual English/Spanish preferred but not requiredBenefits Overview: We offer attractive compensation with comprehensive benefits including: Medical/Dental/Prescription/Vision/Life Insurance; 403(b); Credit Union; FSAs; Short-and-Long-Term Disability; Transportation Plan; Generous Paid Vacations and HolidaysS:US IS AN EQUAL OPPORTUNITY EMPLOYERJoin a team of employees who care about the wellbeing of others.
