Role Description This full-time hybrid role as a Case Manager is based in Braintree, MA, with flexibility for some remote work. The Case Manager will oversee and coordinate care plans for clients, ensuring that they receive quality home care services. Responsibilities include conducting assessments, managing caseloads, collaborating with healthcare providers and families, monitoring care plans, and ensuring compliance with industry regulations. The role will also involve administrative tasks such as maintaining accurate documentation and participating in case reviews to evaluate outcomes. Qualifications Strong case management experience, including care planning, client assessment, and managing caseloadsExcellent communication and interpersonal skills for collaborating with clients, families, and healthcare providersOrganizational and administrative skills, including maintaining detailed documentation and ensuring complianceKnowledge of healthcare regulations and standards, particularly in home care and caregiving programsProficiency in basic computer applications and case management softwareAbility to work independently in a hybrid environment with reliable time managementRelevant certifications, such as a Certified Case Manager (CCM), or eligibility to pursue certifications, is advantageousExperience in home care, nursing, or healthcare coordination is a plus
