API Developer (Node.js + Express)Primary Skill Required for the Role: Javascript Level Required for Primary Skill: Intermediate (3-5 years experience)Remote API Developer (Node.js + Express) Role Description: Backend Node.js API Developer – Specialty Referrals Engagement Context Supports the Primary Care Specialty Referrals workstream, building and maintaining backend services that power referral decisioning workflows, helping primary care providers select the most appropriate specialty for a patient's care needs.Key Responsibilities:Design, build, and maintain RESTful APIs supporting specialty referral decisioning workflows, including specialty recommendation logic, provider/specialist matching, and notificationsImplement and maintain the business rules and decision logic that drive specialty recommendations based on patient and referral dataDesign and optimize database schemas for referral, provider, and status data; write efficient queries and manage migrations as the data model evolvesIntegrate with upstream and downstream systems (EHR, scheduling, provider directory, or prior authorization services as applicable to the workstream)Implement data validation, error handling, and audit logging appropriate for a healthcare production environmentPartner with front-end, integration, and QA teams to deliver end-to-end featuresSupport performance tuning, security hardening, and observability (logging, monitoring, alerting)Participate in code reviews, CI/CD pipeline maintenance, and technical documentationCollaborate with the product/BA team to translate referral decisioning requirements into technical designsRequired Skills:4+ years of professional experience with Node.js and ExpressStrong grasp of RESTful API design, versioning, and JSON schema validationStrong database skills, including schema design, indexing, and query optimization, with hands-on experience in PostgreSQL and tools such as PgAdmin, Prisma, and Liquibase for schema management and migrationsExperience implementing business rules or decision-logic engines within backend servicesExperience with unit and integration testing (Jest, Mocha, or similar)Familiarity with CI/CD tooling (GitHub Actions, GitOps)Experience with cloud-native development; GCP experience preferredPreferred Qualifications:Healthcare payer or provider industry experienceFamiliarity with referral management, specialty recommendation, or prior authorization workflowsExposure to healthcare data standards (HL7, FHIR, or X12 275/278) is a plusPrior experience in a consulting or client-facing delivery environment
