Part-Time Case Manager

Position SummaryThe Part-Time Case Manager is responsible for maintaining consistent engagement with Macon AIM clients and supporting their progress toward established goals. This role ensures participants remain active in the program by providing ongoing communication, accountability, and access to family support resources. This is a grant-funded position through December 2026; should additional funding become available, the role may be extended. Key Responsibilities· Maintain regular contact with assigned clients (via phone, text, email, and in-person as needed)· Monitor and support clients’ progress toward individual goals· Ensure clients are actively participating in required program components, including workshops, events, and activities· Provide information, referrals, and connections to family support services and community resources· Track and document client interactions, attendance, and progress accurately· Conduct follow-ups with clients who miss scheduled events or show decreased engagement· Assist with outreach and engagement efforts to encourage consistent program participation· Assist with planning and supporting resource events as needed· Collaborate and work closely with the Program Director and intern to support program operations and client success· Other duties as assigned Qualifications· Bachelor’s degree required (Social Work, Human Services, Education, or related field preferred)· Experience in case management, social services, education, or a related field preferred· Strong interpersonal and communication skills· Ability to build rapport and maintain professional relationships with diverse populations· Strong organizational skills and attention to detail· Ability to manage time effectively in a part-time role· Basic computer skills (email, data entry, tracking systems)· Passion for supporting families and strengthening communities· Bilingual (Spanish) Preferred Skills· Experience working with families and youth· Knowledge of community resources· Ability to motivate and hold clients accountable in a supportive manner Work Environment· A combination of office, community-based, and virtual communication· Flexible scheduling within the 20-hour work week based on program needs Mission AlignmentThis position supports Macon AIM’s commitment to empowering families, removing barriers, and building stronger futures through consistent engagement and supportive services.
