Job Description Job Summary: Seeking a qualified Physical Therapist to provide rehabilitation services in a skilled nursing facility. The therapist will assess patient needs, develop treatment plans, and deliver therapies to improve mobility and function. Responsibilities:Evaluate patients to determine their physical therapy needs and develop personalized care plans.Implement therapeutic interventions to enhance mobility, strength, and overall health.Collaborate with nursing staff and administrative teams to ensure quality patient care.Document patient progress and adjust treatments accordingly to meet recovery goals.Educate patients and families on therapy routines and post-discharge care. Qualifications:Valid state-issued Physical Therapist license.Proven experience working in a SNF, post-hospital, or rehab environment.Strong assessment and treatment planning skills.Excellent communication and teamwork abilities.Ability to adapt therapy approaches to individual patient needs.
