RN Case Manager

Job Overview We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will have experience in various clinical settings and possess strong patient care and management skills. Job Details Shift- Day Guaranteed Hours- 40/Week Contract Duration- 13 Weeks Responsibilities - Provide comprehensive nursing care to patients in a clinic or outpatient setting - Manage and coordinate patient care plans - Conduct assessments and monitor patient progress - Collaborate with healthcare providers to ensure optimal patient outcomes - Utilize utilization management techniques to optimize healthcare resources - Assist in infectious disease care and treatment - Monitor vital signs and respond to acute care needs Qualifications - Valid California RN license - Experience in geriatrics, Level I Trauma Center, OR, or acute care settings preferred - Strong understanding of utilization management principles - Ability to provide high-quality patient care with compassion and empathy - Proficient in assessing patient needs and developing individualized care plans #HC #Healthcare Job Types: Contract, Travel nursing Pay: $2,450.00 - $2,560.00 per week Experience: Registered Nurse: 2 years (Required) Case management: 2 years (Required) License/Certification: California RN license (Required) Work Location: On the road
