Case Manager

Position Details Role: Patient Care Coordinator (Case Manager) Total Openings: 10 Shift: 8:30 a.m. – 5:00 p.m. Every other weekend required Mid‑week starts can be accommodated if all requirements are cleared Position Description The Patient Care Coordinator (PCC) oversees the management and coordination of care for the acute inpatient population. This role collaborates closely with Hospital-Based Service Physicians, patients and families, nursing, utilization review, and other healthcare team members to ensure continuity of care, safe transitions, and cost-effective outcomes. The PCC also coordinates with community providers and ambulatory case managers to support appropriate discharge planning and prevent readmissions. Key Responsibilities Complete an initial face-to-face assessment for every admitted patient within 24 hours Document assessments in KPHC and communicate outcomes to the physician team and patient/family Review and document discharge plans in compliance with KP, facility, and regulatory requirements Coordinate referrals and communication with in-network vendors for: Home Health DME IV Infusion SNF/Sub‑Acute Acute Rehab 📋 Requirements Associate Degree in Nursing Minimum 1 year experience in: Acute medical/surgical Emergency Department, or Critical care nursing Experience using an electronic medical record (EMR) system Knowledge of healthcare reimbursement systems: Medicare Advantage Commercial payers Medicaid CMS regulatory rules Completion of PCC Assessment of Critical Skills
