This position is ideal forlocal nurseswho are passionate about helping patients regain independence and achieve optimal recovery outcomes. Key ResponsibilitiesProvide skilled nursing care to residents requiring rehabilitation following illness, injury, or surgery. Assess patient conditions, develop individualized care plans, and deliver nursing interventions aligned with rehabilitative goals. Administer medications, treatments, and therapies as ordered. Collaborate closely with physical therapists, occupational therapists, and an interdisciplinary care team. Monitor patient progress and adjust care plans as needed. Document all assessments, interventions, and changes in condition accurately and timely. Maintain a safe, supportive environment that promotes healing and resident engagement. Communicate effectively with patients, families, and healthcare staff. RequirementsActive RN licensein the state. Current CPR and First Aid certifications(required). Experience in rehabilitation, skilled nursing, or post-acute care preferred. Strong clinical assessment, coordination, and communication skills.
