Physical Therapist Location: Lowell, MA Agency: Cynet Health Pay: $2,528 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 6/22/2026 About the Position Job Title: Physical Therapist Profession: Physical Therapist Specialty: Long Term Care Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: Minimum 1 year of PT experience, LTC or rehab experience License: Massachusetts State License Certifications: None specified Must-Have: Must have Level II experience if a new graduate Description: Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care. Develops appropriate plan of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practices to support clinical interventions. Administers various procedures as part of the rehabilitation plan, including but not limited to manual techniques, ambulation, therapeutic exercises, modalities, and the use of supportive and assistive devices. Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness. Modifications to services are made in relation to patient evaluation, and referrals are made based on outcomes. Provides consultation and counseling to patients, family members, caregivers, and other service providers regarding physical disorders. Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services rendered per regulatory and clinical requirements. Documents all required information in the electronic documentation system and maintains patient records according to policy. Accurately records treatment minutes for services provided per regulatory and clinical requirements. Participates in required meetings, including but not limited to patient care conferences and rehabilitation meetings. Participates in facility and company-required trainings, in-services, and conferences. Able to obtain any necessary Durable Medical Equipment or supplies per regulatory and clinical practice requirements. Able to supervise and support physical therapist assistants, rehabilitation aides, and/or students.
