The Registered Nurse will assess, plan, implement and evaluate care based on age-specific components while assuming responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.A Career At VA Salt Lake City Health Care System Can Take You Places (youtube.com)Duties and responsibilities include, but are not limited to the following:Responsible and accountable for individual nursing practice and will determine the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care.Provide sex-specific direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Primary Care Department.Manage assigned panel(s), utilize registries, and ensure appropriate evaluation and access is provided to patient panels.Provide health promotion, health education and coaching on wellness, disease prevention, and chronic care management.Focus on individual growth and development in outpatient PACT nursing practice with increased levels of self-direction.Ability to formulate a plan of care based on nursing assessments.Seek guidance on how to improve care provided to patients and families.Delegate elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient.Bring primary care principles and theory with limited application of critical thinking skills.Provide a full range of nursing care to Veteran/Patients, including complex care needs using the nursing process.Implement the PACT concept of right person, right place, right time that ensures access to care and supports staff functioning at the top of their scope.Management of Veteran chronic disease conditions through ongoing assessment of conditions and recognizing changes in condition and responding with timely and correct interventions.Establish plan of care and goals with Veteran Patient after assessing learning needs, learning preferences, readiness to learn and modify modalities of teaching when needed.Initiate referrals to other disciplines and services as appropriate and allowed by current Nursing Protocols.Offer preventative health measures to Veterans to include Whole Health modalities.Apply all modalities of care available to deliver best practice to panel.Ensure that documentation of care is complete, accurate, consistent, concise, and timely. Encounters will coded appropriately and timely. Documentation will be alerted to other team members as needed. Recommend use of SOAP/SBAR models for documentation.Implement appropriate notification processes for problematic events involving patient, family, and staff along with appropriate documentation as needed.Utilize tools and reports to provide proactive management of Veterans with chronic disease or at high risk.Promote, advocate for, and strive to protect the health, safety and rights of the patient.Demonstrate responsibility and accountability for own nursing judgements and actions.Collaborate with healthcare consumer and other key stakeholders in the conduct of nursing practice.Develop and implement a plan to meet learning needs, instructing patients and families in basic health teaching.Ensure application of institutional policy to protect information from unauthorized release, loss, alteration, or deletion.Maintain competency on PPE use/donning and doffing.Comply with safety rules and regulations for safe job performance and fire protection. Practice safe body mechanics, use mechanical devices, and/or request help when needed in moving, lifting, or turning patients to prevent injury to patient, self or others. Promptly report all accidents t supervisor and notify supervisor promptly of unsafe or unhealthy environmental situations/conditions.Responsible for environment of care (EOC) issues when on duty.
