Licensed Practical NurseThe licensed practical nurse is responsible for making decisions that are based on the individual's educational preparation and nursing experience, and for providing nursing services as ordered on patient plan of care. Reports problems/potential problems to RN/Case Manager.Duties And Responsibilities:Participation in the nursing process by providing input to the clients plan of careImplementing the care plan as writtenEvaluating effectiveness of care plan, make recommendations for revisions as appropriate.Records all pertinent observations and treatments as per agency policies. Prepares clinical progress notes and provide appropriate documentation of service in keeping with agency policies and procedures.Provide for the emotional and physical comfort and safety of the clientPerform nursing procedures for which he/she has been trainedNotify appropriate person about unusual symptoms and/or changes in conditions of clientsParticipate in case conferences, team meetings and performance improvement activities as assignedProvides proof of current license by the state. Provides necessary information to verify education and work experiencesParticipates in in-service programsPractices safely and competently within the job descriptionComplies with all state regulatory actsComplies with all agency policies and procedures including HIPAA Privacy RulesProvides documentation of high tech certification in appropriate; performs duties as qualified/authorizedMaintains patient/business confidentialityQualifications:Must be a graduate of an approved school of nursing and currently licensed in the state of the HHA. Must have one year current experience in accredited hospital or accredited nursing home within two years previous to employment.
