Key Responsibilities: Provide integrated services that are recovery-focused, culturally competent, and patient-centered. Collaborate within a multidisciplinary team to support health behavior change. Conduct comprehensive assessments and develop tailored care management plans. Advocate for clients' needs including healthcare benefits, housing, education, and employment. Coordinate care across various treatment settings including office and community-based settings and provide crisis management when needed. ODMHSAS Case Management certification or certified within 60 days of hire. Experience in mental health or healthcare settings preferred. Strong communication skills and proficiency in MS Word and general computer use. Access to reliable transportation, valid Oklahoma drivers license, and automobile liability insurance.
