Lead Care Manager Bridge to Care Program | Operations DepartmentReports to: Program Director Type: Part-Time, 20 hrs/week | Non-Exempt Pay: $26.00 - $28.00/hourSetting: Fully remote + field-based (San Diego County)About the RoleFundaMental Health is seeking a compassionate, detail-oriented Lead Care Manager (LCM) to deliver Enhanced Care Management (ECM) services to high-need Medi-Cal members in San Diego County. The LCM serves as the primary point of care coordination for assigned members, providing outreach, assessment, person-centered care planning, and ongoing coordination across medical, behavioral health, and social service systems.This is a remote, field-based role supporting individuals with complex needs, including homelessness, serious mental illness, and substance use disorders. Direct service is delivered in community settings; all meetings, supervision, and documentation are completed remotely.Core ResponsibilitiesDeliver ECM services in compliance with CalAIM, DHCS, and Managed Care Plan requirementsConduct outreach and engagement via in-person visits, phone, text, and telehealthComplete ECM Comprehensive Assessments covering medical, behavioral health, substance use, and social determinants of healthDevelop and maintain individualized, person-centered Care PlansServe as the primary point of contact for members, their supports, and the interdisciplinary care teamCoordinate care across primary care, behavioral health, hospitals, housing, and community organizationsSupport transitions of care (hospital discharge, housing changes, institutional transitions)Advocate for members to reduce barriers and improve access to servicesMaintain accurate, audit-ready documentation (assessments, care plans, progress notes, encounter records)Participate in virtual team meetings, case conferences, trainings, and supervisionFollow field safety, crisis response, and incident reporting protocolsFulfill mandated reporting duties per California lawAdhere to HIPAA, confidentiality, and fraud, waste, and abuse (FWA) prevention standardsQualificationsRequired:High school diploma or GEDMinimum 1 year of experience in care management, care coordination, or related servicesExperience with underserved or high-acuity populations (mental health, substance use, homelessness, justice involvement)Ability to work safely in community/field settings, including de-escalation and crisis responseStrong interpersonal, communication, and organizational skillsProficiency with electronic documentation systems and Google WorkspaceReliable transportation, valid CA driver's license, and active auto insuranceAbility to pass required background checks and health screeningsPreferred:Bachelor's degree in Social Work, Public Health, Psychology, Human Services, or related fieldPrior experience in ECM, Whole Person Care, Health Homes, or CalAIM-funded programsBilingual in Spanish or another threshold languageFamiliarity with San Diego County health, behavioral health, housing, and social service systemsTraining in trauma-informed care, motivational interviewing, or harm reductionWork EnvironmentRemote-first with no in-office reporting location. Home office used for administrative work; fieldwork conducted independently across San Diego County (homes, shelters, clinics, partner organizations). Reliable internet and the ability to work autonomously are essential.
