Job Opportunity: Home Health NurseAs a Home Health Nurse, you will be responsible for providing high-quality patient care in the comfort of their own homes. You will work with a multidisciplinary team to assess patients' needs and develop personalized care plans.Key Responsibilities: Conduct home visits to assess patients' physical and emotional needs. Develop and implement individualized care plans that meet each patient's unique needs and goals. Work collaboratively with other healthcare professionals to ensure comprehensive care. Educate patients and their families on disease management, treatment options, and health maintenance. Monitor patients' progress, report changes to physicians, and adjust care plans as needed. Requirements: Graduate of an accredited Registered Nurse (RN) program. Active, unrestricted Registered Nurse (RN) license in the State of South Carolina or compact multi-state license.
