This role offers autonomy, strong team support, and meaningful patient relationships.Schedule (Full-Time, Exempt)Saturday & Sunday: 7:00 AM - 8:00 PMMonday: 4 hours for follow-ups and documentationKey ResponsibilitiesPerform patient assessments and develop individualized care plansDeliver skilled nursing care, including medication and treatment managementMonitor patient progress and adjust care as neededEducate patients and caregivers on health management and self-careCoordinate with physicians and interdisciplinary teamsSupervise Home Health Aides and LPNsComplete timely and accurate clinical documentation (including OASIS)QualificationsActive RN licenseBSN + 1 year experience OR ASN/Diploma + 2 years experienceCurrent CPR certificationValid driver's license and reliable transportationPreferredHome health experienceBehavioral or mental health experiencePhysical Demands & Work EnvironmentFrequent standing, walking, sitting, and use of handsRegular bending, kneeling, and reachingOccasional lifting and repositioning of patientsAbility to lift/push up to and over 25 lbs as neededExposure to clinical environments and infectious materialsJoin a team where you can deliver meaningful, patient-centered care-right where it matters most.
