Role: Patient Care CoordinatorLocation - Texas - RemoteType - 6 months + /conversion to permPosition PurposeThe Care Manager develops, assesses, and coordinates care management activities to deliver high-quality, cost-effective healthcare outcomes for members. This role creates personalized care plans, educates members and their families on available services and benefits, and provides advocacy and coordination to ensure appropriate and timely access to care.Key Responsibilities:Evaluate member needs, identify barriers to care, and recommend solutions to achieve the best possible outcomesDevelop and maintain personalized care plans/service plans in collaboration with members, families, and caregiversCoordinate services between members, families, healthcare providers, specialists, and community resourcesProvide psychosocial support and connect members to resources such as housing, food, employment, education, and transportationMonitor progress toward care plan goals and collaborate with providers to adjust plans as neededEnsure timely access to needed care, services, and referralsDocument all care management activities to maintain compliance with state, federal, and payer regulationsConduct on-site visits when appropriate to assess member needsEducate members, families, and care teams on healthcare options, benefits, and proceduresEducation & Experience:Bachelor’s degree required (or equivalent combination of education and experience)2–4 years of experience in care management, case management, or related healthcare roleClinical license (RN, LPN, etc.) is preferred but not requiredExperience working with Medicaid, vulnerable populations, or managed care is strongly preferred
