The Social Worker, LICSW, Mental Health Counselor LMHC, or Marriage and Family Therapist LMFT will function as a core member of a multidisciplinary team and communicate patient's health care needs with primary care, behavioral health services, health care facilities and/or payers. Acts as an advocate and helps patient gain access to resources while confronting their personal issues, such as mental illness, addiction, and abuse. The position takes responsibility for management of a therapeutic caseload (individual and family therapy to children, adolescents, and adults as requested) providing timely charting and record keeping.
