If you’re passionate about making a difference and thrive in a collaborative setting, New Bridge Medical Center is looking for a Patient Navigator in the LGBTQ+ Health Center.RequirementsPatient Intake & Scheduling: Schedules patients for visits within the program, greets them arriving for appointments, and provides clear details regarding directions and visit expectations.Medication & Care Tracking: Maintains a secure program spreadsheet to track upcoming visit timelines and remaining medication refills; proactively calling patients to ensure continuity of care.Surgical Process Navigation and Clearances: Supports patients seeking surgeries, particularly gender-affirming procedures, by managing complex pre-surgical requirements and clinical medical documentation.Pre-Authorization Management: Initiates, submits, and tracks pre-authorization requests to insurance companies for prescribed treatments and medications, providing all required documentation.Insurance Advocacy: Proactively follows up on pending or expired pre-authorizations, communicating directly with insurance companies to resolve denials, complex cases, or delays.Financial Support & Counseling: Provides comprehensive financial counseling to help uninsured or underinsured patients explore assistance programs, manage out-of-pocket costs, and access care.Clinical Screening & Education: Delivers health education and conduct screenings for psychosocial needs as directed by the physician or clinical providers.Resource Referral & Follow-Up: Provides referrals to internal and community resources, conducting direct follow-up to ensure successful patient engagement and referral completion.Community Outreach: Conducts outreach on behalf of the LGBTQ+ Health and Wellness Center, including attending community events to promote clinical services and build regional partnerships.Other DutiesYou'll also help with other tasks as needed to support our department and clinic goals.Position Qualifications Education: Bachelor’s Degree Required, degree in Public Health and/or Social Work preferred.Experience:Previous experience working in patient navigation, case management, or the medical field is preferred.Experience with or strong understanding of the LGBTQ+ community's healthcare needs.Skills & Core Competencies:Strong communication skills and people skills to work with patients and our team.Strong problem-solving skills for navigating challenges.Good time management is important to prioritize tasks and meet deadlines.Ability to work independently and as part of a team.A positive, professional attitude that helps build a strong team.Experience with Microsoft Office, especially MS Excel.Spanish proficiency is preferredSalary Commensurate With Experience Within Posted Range$42K - $62K annuallyWe provide a comprehensive benefits package, including a competitive medical, dental, and vision plans.
