VACCHCS is a 57-bed acute care, general medical and surgical center with state-of-the-art primary, secondary, and tertiary care in major diagnostic and treatment specializations. A 54-bed Community Living Center (CLC) provides skilled nursing and rehabilitation care.Location: 2615 E. Clinton Ave, Fresno CA 93703Essential Duties and ResponsibilitiesCoordinate all aspects of care for hospitalized medical patients including:Admission, including history and physical examinationsAppropriate placement within the hospitalEvaluation and treatment, including the ordering and interpretation of diagnostic studies, and the administration of medicationsDiscussion of end of life preferences/advanced care planningConsultations with sub-specialistsTimely and proactive discharge planningCoordination of care with primary care physician and discharge including ensuring appropriate follow-up is arrangedThis may include perioperative care but generally excludes preoperative assessments of patients for determination of fitness for surgery.Contractor's physicians shall assess, stabilize, and determine the disposition of inpatients with emergent conditions consistent with medical staff policy regarding emergency and consultative call services.
