Care Coordinator – Children's Long-Term Support (CLTS) ProgramWaukesha County, WIFull-Time | M–F, First Shift | Remote Flexibility$23.70/hrLutheran Social Services of Wisconsin & Upper Michigan is seeking a compassionate and organized Care Coordinator to join our CLTS Waiver Program team in Waukesha County!The Children's Long-Term Support (CLTS) Waiver Program is a federally funded Medicaid initiative that helps children with developmental, physical, or severe emotional disabilities receive services that allow them to thrive in home and community settings.
