Company Description**$2,000 Sign on bonus!** Flexible hours** Monday through Friday schedule** Small and collaborative teamWelcome Family Springfield Holyoke is looking for a part-time (with the potential for increased hours) Maternal Child Health Nurses to offer universal, one-time home visits to moms and newborns in Springfield, Holyoke, and the surrounding communities. Visits are provided in-person or virtually by parent preference, up to 8 weeks following delivery. Welcome Family partners with Baystate's Babies at the Wesson Women & Infants' Unit, Baystate Medical Center.Job DescriptionCollaborate with referral sourcesProvide outreach to potential participantsSchedule and conduct up to 90-minute visits at the family's home or by telehealthScreen in six core areas: clinical assessment, unmet health needs, maternal and infant nutrition, substance use, emotional health, and domestic violenceProvide brief intervention and education to all families, regardless of perceived risk or needAddress the caregiver's individual questions and concernsOffer referrals to services for caregivers with an identified need, including MA MIECHV-funded home visiting programs and other community resources that may best suit the needs of the familyEngage, assess, and present information in an unbiased, culturally appropriate manner, particularly related to sensitive topicsConduct a follow up contact with all caregivers 2-3 weeks post initial visitCollect data using the Welcome Family forms, complete required DPH and agency documentation and billingParticipate in the Welcome Family Learning Collaborative and DPH annual site visitComplete agency and state required trainingQualificationsCurrent licensure as an RN by the Massachusetts Board of Registration, Division of Professional Licensure, with either:Bachelor's Degree in Nursing or Master of Science Degree in Nursing in Maternal and Child Health, Family Health, Community Health, or related specialtyTwo or more years of professional work experience with prenatal, newborns, pediatrics, lactation, or maternal servicesProficiency in Spanish preferredCLC Certification requiredAdditional Information
