Your Impact & Responsibilities: Bedside Excellence: Provide high-quality nursing care, including medication administration, wound treatments, and resident assessments. Clinical Monitoring: Closely monitor resident vitals and conditions, promptly reporting any changes to the RN or physician. Care Coordination: Collaborate with our in-house therapy team (PT/OT/SLP) and physicians to ensure comprehensive recovery plans. Documentation: Maintain precise, timely, and compliant electronic medical records (PointClickCare experience is highly valued). Leadership: Provide direction and mentorship to CNAs to ensure resident ADLs are met with the highest standards of care. Advocacy: Act as a compassionate resource for residents and their families during their transitional care journey. Qualifications: License: Must possess a current, valid California Licensed Vocational Nurse (LVN) license. Certification: Current CPR/BLS Certification. Experience: Previous experience in subacute, skilled nursing, or transitional care is preferred. Skillset: Strong clinical judgment, excellent communication skills, and a "team-first" attitude.
