Job Description JOBID: 885856Position: PT Home HealthShift Notes Mon-Fri, 8a-5p 40 hours per weekLocation : Breckenridge, MN, 56520-1108Job Type: 13 weeksStart date:06/16/2025 Required Certifications BLSPay:$50-$53Unit PT-Home Health Required Certifications Physical Therapy licensure in MN and ND. Common diagnosis / Types of patients/patient demographics - Adults, fx's, weakness, new joints, CVA follow up. Home exercise programs Additional Job Duties/Utilization Review involved- Case conferencing for team collaboration RT Equipment (vents, Cpap, BiPap, etc) Experience or 'Must have' skills Home Health experience preferred, not required. How many patients do they see per day 4-5/ day. Start of care may be requested. Reevaluations every 30 days, revisits, recertifications when indicated. Visits completed on tablet. Manager/Supervisor Supervisor available 24/7 by phone when needed. Home Health/Hospice Aids? Supervision of aide staff may be required. Shifts & Scheduling (Do you allow self-scheduling= Block scheduling) Self scheduling expect Ped according to provider orders.
