This role partners closely with patients, caregivers, physicians, and the interdisciplinary care team to develop and coordinate individualized plans of care.Key ResponsibilitiesAssess patient and family needs and develop individualized plans of careProvide counseling, behavioral health interventions, education, and community resource supportCoordinate care with physicians and interdisciplinary team membersDocument patient progress, interventions, and outcomes in accordance with agency standardsConduct hospice information visits for home care patients when requestedSupport patient and caregiver education and discharge planningMentor and support new MSW staff and studentsBring your care where it matters most-home.
