The Organization: Hand in Hand of Glynn, Inc. was organized as a 501c3 in July 2018 to help end chronic homelessness in Glynn County. We have built 60 Tiny Homes to house those currently experiencing homelessness. We provide on-site services and classes at our 8,000-square-foot Community Center to help our residents return to wellness and a productive life. We teach life skills including nutrition, cooking, exercise, scheduling, financial skills, as well as light technical training and computer skills.
We will offer medical and psychological counseling and have a satellite clinic staffed by Coastal Community Health two days a week. Neighborhood AA meetings and GED classes will be scheduled along with computer basics and financial acumen.
Mission: Our goal is to house 60 people who are now homeless; to change the trajectory of 60 lives; and to bring dignity and self-respect back to those who have lost their connection to their friends, family, and community. Hand in Hand is a member of the Homeless Coalition of Glynn County, Ga. whose member organizations address various aspects of homelessness in our community. (i.e. Safe Harbor, The Well, Manna House, Saved by Grace, Second Harvest, etc.)
Yet with all these services provided,Hand in Hand of Glynn isunique in that we offer Permanent Supportive Affordable Housing with 60 Tiny Homes (240 Sq. Ft. each) to those currently experiencing homelessness. We believe in “Housing First” as a proven remedy for those individuals experiencing homelessness, and especially the chronically homeless.
Vision: “We believe housing is a right, not a reward for a positive clinical outcome.” “Housing First” means once you are “housed” in a safe, dry place of your own, you can focus on and tackle other issues of health and well-being that you may be facing. We will offer a supportive community, case workers and medical and psychological counseling services to help our residents make healthy choices for themselves to regain their footing and their health and to begin a new life in their new home.

Identifying, initiating interviews, selection, and onboarding. Setting up initial intake interviews.
Scheduled Hours: 8:30 a.m. to 5:00 p.m. This is a management position that requires frequent overtime as part of the position responsibilities.
Responsibilities:
The Resident Care Coordinator will need to supply case management to the 60 residents including signing them up to be assessed by Coastal Community Health or whomever is supplying our healthcare for whatever physical and mental assistance is needed. He or she will need to assist our residents in applying for financial support for those services (Medicaid or Medicare), getting their basic identification established including driver’s License or the equivalent Identification papers, securing birth certificates, etc. so that our residents can receive services and participate in available programs.
The Resident Care Coordinator is responsible for all activities around “on-boarding” our residents: Making sure they shower and have fresh clothes and that their belongings go through a de-bugging process. Our Coordinator of Volunteers may also be helpful in this activity, but it is the responsibility of the Resident Care Coordinator to insure the successful on- Boarding of our residents.
All documentation of the resident must be maintained by The Resident Care Coordinator (RCC). This includes their name and any phone numbers in case of emergency or if we need to contact a next of kin. We also need a copy of the agreement they signed with the Brunswick Housing Authority and their lease agreement with The Grove at Correll Commons. It is essential that the residents understand their lease agreement with The Grove and that they are aware of their financial responsibilities in order to stay in our program.
Regular individual meetings with residents are required for the Resident Care Coordinator to establish rapport and gain trust and credibility with each and every resident, and to insure that the residents understand all policies and regulationsof the Community. Meeting every resident at least twice a month is the goal. Repetition of the rules and regulations are needed but also needed is setting individual goals and plans for the future and helping instruct our residents in the use of calendars and planning a schedule for them to follow. Making residents aware of the programs and service available is the responsibility of the RCC.
Monitoring residents to insure their mental and physical well being: The Resident Care Coordinator keeps tabs on residents. Along with the Program Director, the RCC meets with the residents as a group once every week at first, going to bi-monthly meetings after the first six months of residency. Meetings are for the residents to talk and give suggestions and /or voice complaints or needs if they have them. We are there to hear their suggestions but also, it is the Resident Care Coordinator’s responsibility to address any problems with the Director or other responsible party that might surface during conversations with a given resident and to appropriately counsel residents about their own behavior.
Encourages residents to participate: Residents are encouraged by the Resident Care Coordinator to sign up for classes and services that the Resident Care Coordinator believes would be beneficial. If a given class or activity would be particularly beneficial for a given resident, the RCC may need to be persuasive in order for the resident to understand the advantages of taking the class or participating in the program. But it is up to the resident to participate. The RCC can point out the benefits of a given class or activity but cannot coerce the resident to participate. Nonetheless, the amount of trust and rapport that has been established with the resident should prove helpful in persuading a given resident to participate in a needed program, service or class.
Case management experience is highly desired and obviously, very helpful. Work with individuals in our population or a similar demographic is highly desired but the ability to connect individuals to resources available is essential.
Teaching capabilities would be a great added benefit in the successful candidate because the RCC might be able to teach some of the basic life skill classes. (Nutrition, wellness, cleanliness, housekeeping, Etc.)
Salary will be commensurate with education and experience.
