This role focuses on preventive care, chronic disease management, and acute care while working closely with a multidisciplinary team to deliver high-quality, community-centered healthcare.Job Duties & Responsibilities:Provide full-spectrum family medicine care to pediatric, adult, and geriatric patientsDiagnose and manage acute and chronic medical conditionsConduct preventive care visits and routine wellness examsProvide inpatient care as part of an integrated care modelOrder, interpret, and follow up on diagnostic tests and imagingDevelop and manage treatment plans and medication regimensCoordinate referrals and collaborate with specialists as neededMaintain accurate and timely documentation in the EHRParticipate in team meetings and quality improvement initiativesSupport continuity of care and positive patient experiencesPrerequisites / License & Certification Requirements:MD or DO degree from an accredited medical schoolBoard certification in Family MedicineActive, unrestricted Arizona medical licenseEligible for hospital privilegesExperience in outpatient family medicine preferredIf you meet all of our criteria and would like to be considered, please apply with your most updated Resume/CV.
