About the Primary Care Medical Assistant Role
Upperline Health Plus is seeking a Primary Care Medical Assistant who offers high quality, patient centered care in a value-based practice.
In our patient-centered and outcome driven environment the Medical Assistant supports patients in office with duties in-office such as rooming patients, obtaining medical history and taking vital signs as well as ensuring care beyond the moment by coordinating care, supporting extended care team efforts and conducting proactive outreach to improve outcomes and patient experience. The MA also provides direct support to the Upperline Plus Primary Care Provider to ensure smooth clinical operations and effective team-based care.

This role will be on-site in our Lithonia, GA office.

Primary Care Medical Assistant Responsibilities
Room patients, obtain and document vital signs, medical history, and chief complaints
Assist NP with health assessments and in clinic visits with Upperline Plus patients being seen in our clinics
Facilitate and support virtual provider encounters from the clinic setting
Accurately document clinical information in the electronic health record (EHR)
Performs thorough and accurate medication reconciliation at each patient encounter by reviewing, verifying, and consolidating all prescription medications, over-the-counter drugs, supplements, and herbal products
Identify medication discrepancies or potential concerns; and ensures the reconciled medication list is complete, current, and accurately documented in the electronic medical record for provider review
Telephonic outreach and engagement with Upperline Plus patients to coordinate ongoing preventative care
Manage in-basket messages, prescription refill requests, and prior authorizations
Care coordination activities as assigned including, but not limited to, medical records requests, order management, appointment scheduling, managing patient care referrals, DME referrals, etc.
Identifying community resources and assisting Upperline's patients and families in accessing those resources
Prepare and maintain equipment and supplies commonly used by the Upperline Plus provider
Collaborate with providers, nurses, care managers, and social services to address medical and social determinants of health
Serve as the Upperline Plus provider's primary point of support for workflow optimization and patient flow management
Identify barriers to care (transportation, medication access, health literacy) and escalate appropriately
Participation in interdisciplinary team meetings & daily clinic team huddles
Perform clinical tasks within scope (injections, phlebotomy, point-of-care testing)
Other related duties as deemed necessary by Upperline Health
Primary Care Medical Assistant Qualifications
Certified MA or minimum of 2 years' experience in a medical primary care setting
Experience in a primary care /outpatient clinic setting is strongly preferred
Certified MA or phlebotomy certification strongly preferred
Demonstrates strong knowledge of medical terminology, disease processes, and basic pharmacology, including medication classifications, common indications, dosing ranges, and potential side effects
Ability to accurately interpret provider documentation, diagnostic orders, and treatment plans
Clinical care coordination or care management experience is ideal
Previous experience assisting or providing care to the adultgeriatric patient population is preferred
Prior wound care and foot care experience is a plus
Bilingual English/Spanish is helpful
Ability to work collaboratively in a team-based care model
Previous experience working in an EMR systemideally, AthenaHealth
Proficiency using Word, Excel and other Microsoft Office products is necessary
Must have reliable transportation, valid driver's license and minimum state required liability auto insurance
Extremely strong customer-service orientation and work ethic
Comfort managing multiple tasks and prioritizing in a fast-paced environment
Empathy, professionalism, and cultural sensitivity
Self-starter with an entrepreneurial spirit and demonstrated problem solving skills
Excellent oral and written communication skills to ensure that each of your interactions with patients, family caregivers, clinicians, and team members is effective and meaningful
Compensation
Compensation is commensurate to compensation for similar positions in the region and based on prior training and experience.
