This position is classified as Non-Exempt and reports to the Director of Rehabilitation and the Facility Administrator.Core responsibilities include:Performing comprehensive evaluations and reassessments in accordance with physician orders and California standards of practiceDeveloping, implementing, and modifying individualized plans of care with measurable functional goalsDelivering skilled therapeutic interventions, both individual and group, to restore function and reduce impairmentMonitoring patient progress and adjusting treatment interventions based on clinical presentation and objective outcomesSupervising and directing Physical Therapist Assistants in accordance with California state law and professional regulationsProviding appropriate oversight to rehabilitation aides, students, and volunteers as assignedCommunicating evaluation findings, progress updates, and discharge outcomes to physicians, interdisciplinary team members, patients, and familiesDeveloping and instructing patients and caregivers in home exercise programs, safe use of assistive devices, orthotics, and adaptive equipmentCompleting timely, accurate, and compliant documentation consistent with federal, state, payer, and organizational requirementsParticipating in MDS assessments, care conferences, care plan development, and quarterly reviewsSupporting discharge planning, transition-of-care coordination, and community reintegration recommendationsParticipating in quality assurance, performance improvement, utilization review, and risk management initiativesMaintaining compliance with HIPAA, patient rights, OSHA, infection control, and all applicable California state practice standardsTreating all residents with dignity, respect, and compassionate professionalismPhysical requirements include the ability to assist with patient transfers, mobility support, repositioning, prolonged standing and walking, bending, reaching, and lifting with or without assistive devices. The ability to assist in resident evacuation during emergency situations is also required.What You BringCurrent, active, and unrestricted Physical Therapist license in the State of California — required (pending licensure considered)Graduate of an accredited Physical Therapy program (Bachelor's, Master's, or Doctorate) — requiredSuccessful completion of the National Physical Therapy Examination (NPTE) — requiredCurrent CPR/BLS certification (or willingness to obtain per facility policy)Experience in a skilled nursing, post-acute, hospital, or related medical setting — strongly preferredTraining in rehabilitative and restorative therapy practicesExcellent verbal and written communication skills with the ability to exercise sound clinical judgmentComposure and professionalism in dynamic, fast-paced clinical environmentsStrong organizational skills, leadership ability, and capacity to make independent clinical decisionsA team-first mindset and a genuine passion for geriatric and post-acute careEqual Opportunity and AccommodationKei-Ai Los Angeles Healthcare Center is proud to be an Equal Opportunity Employer.
