The LCSW delivers evidence-based therapy, coordinates care, and supports patients through recovery in collaboration with an interdisciplinary team.Services are provided in accordance with Indiana licensure laws, NASW Code of Ethics, and organizational policies.Key ResponsibilitiesClinical Assessment & DiagnosisConduct comprehensive biopsychosocial assessmentsEvaluate mental health, substance use, and social determinants of healthDiagnose behavioral health and co-occurring disorders using DSM-5 criteriaAssess risk factors including suicidality, abuse/neglect, and relapse potentialTreatment PlanningDevelop individualized treatment plans with measurable goalsImplement person-centered and strengths-based interventionsMonitor and update treatment plans based on patient progressTherapeutic ServicesProvide:Individual therapyGroup therapyFamily counseling (as appropriate)Apply evidence-based modalities such as:Cognitive Behavioral Therapy (CBT)Motivational Interviewing (MI)Trauma-informed careSolution-Focused TherapyAddress issues including trauma, addiction, family systems, and psychosocial stressorsCare Coordination & Case ManagementCoordinate care with:PhysiciansAPRNsNursesTherapists and counselorsCommunity providersAssist with discharge planning and transitions to appropriate levels of careConnect patients with community resources (housing, employment, financial support)Documentation & ComplianceMaintain accurate, timely documentation in EMR, including:AssessmentsProgress notesTreatment plansDischarge summariesEnsure compliance with:Indiana regulationsHIPAA and 42 CFR Part 2Accreditation standards (Joint Commission/CARF, if applicable)Participate in audits, chart reviews, and quality improvement initiativesCrisis InterventionRespond to behavioral health crises, including:Suicidal or homicidal ideationPsychiatric instabilityAbuse or neglect concernsConduct risk assessments and initiate appropriate interventions or referralsPatient Advocacy & SupportAdvocate for patient access to care and resourcesProvide psychoeducation to patients and familiesPromote trauma-informed, culturally competent careSupport a safe, structured, and therapeutic environmentQualificationsRequiredActive Indiana LCSW license (Licensed Clinical Social Worker)Master’s degree in Social Work (MSW) from an accredited programExperience in:Behavioral healthSubstance use disorders and/or co-occurring disordersKnowledge of DSM-5 and evidence-based therapy practicesPreferredExperience in:Detox, residential, or inpatient settingsCrisis intervention or acute care environmentsFamiliarity with:ASAM criteriaCIWA / COWS withdrawal protocolsExperience with trauma-informed and integrated care modelsSkills & CompetenciesStrong clinical assessment and diagnostic abilitiesKnowledge of social determinants of health and case managementEffective communication and interdisciplinary collaborationCrisis management and de-escalation skillsHigh attention to detail in documentation and compliance
