What You’ll DoDuring a 60-minute in-home visit, you will:Perform comprehensive adult and geriatric assessmentsReview medical history and complete medication reconciliationIdentify chronic conditions and care gapsConduct preventive screenings and point-of-care testing (as applicable)Provide patient education and wellness counselingDocument findings accurately in EMRCollaborate with PCPs and care teams when neededThis role does NOT require managing ongoing primary care panels.????
